Chronic cerebrospinal venous insufficiency in amyotrophic lateral sclerosis

Aim. CCSVI probably is not only a risk factor for development of MS, but it is the basic ethiopathogenetical factor. The dysfunction of the blood brain barrier (BBB) appeared as a result of CCSVI can also be one of the main reasons for triggering of neurodegenerative or autoimmune processes in MS. Following this hypothesis we can assume that CCSVI is the main ethiopathogenetic factor in other neurodegenerative diseases such as amyotrophic lateral sclerosis (ALS). Up to this moment the literature has no data for Doppler or Venography examination for CCSVI in patients with ASL. The aim of this research is screening of ALS patients for CCSVI using EchoDoppler and venography.
Methods. Seven patients with ALS were examined.
Results. The patients diagnose was confirmed based on clinical symptoms and electromyography. Both jugular vein (draining the blood from the brain) and the azygos vein (draining the blood from the spinal cord) were examined by EchoDoppler and venography. In all of the patients with ALS CCSVI was determined- severe stenosis in jugular veins. In five of the patients was determined severe stenosis in the azygos vein. In all of our patients endovascular therapy was performed- balloon dilatation of jugular and azygos vein with good angiographic result- normal blood flow and diameter of the veins.
Conclusion. The discovery of CCSVI in seven patients with ALS is probably not a coincidence and raises the necessity of conduction of a systematic research for CCSVI in of patients with ALS.

Obiettivo.
CCSVI NON E' PROBABILMENTE SOLO UN FATTORE DI RISCHIO PER LO SVILUPPO DI MS , MA E' IL FATTORE FONDAMENTALE ETHIOPATHOGENETICAL.
La Disfunzione della Barriera Ematoencefalica (BBB) Appariva come una Conseguenza di CCSVI può anche Essere uno dei Motivi PRINCIPALI per l'Attivazione di Processi Neurodegenerativi o autoimmune in MS.
Seguendo questa Ipotesi si può Supporre che CCSVI è il fattore PRINCIPALE Ethiopathogenetic in ALTRE MALATTIE NEURODEGENERATIVE come la Sclerosi Laterale Amiotrofica (SLA).
Fino a questo momento la letteratura non ha dati per un esame Doppler o Venografia per CCSVI in pazienti con ASL.
Lo scopo di questa ricerca è lo screening dei pazienti affetti da SLA per CCSVI mediante ecodoppler e flebografia.
metodi.
Sette pazienti affetti da SLA sono stati esaminati.
risultati.
I pazienti di diagnostica è stata confermata sulla base dei sintomi clinici e elettromiografia.
Sia la vena giugulare (drenare il sangue dal cervello) e la vena azygos (drenare il sangue dal midollo spinale) sono stati esaminati mediante ecodoppler e flebografia.
In tutti i pazienti affetti da SLA CCSVI è stata determinata, grave stenosi nelle vene giugulari.
In cinque dei pazienti è stato determinato in grave stenosi della vena azygos.
In tutti i pazienti della nostra terapia endovascolare era eseguita-pallone di dilatazione vena giugulare e azygos con buona angiografica conseguenza-normale flusso sanguigno e diametro delle vene.
C O N C L U S I O N E :
La scoperta della CCSVI in sette pazienti affetti da SLA non è probabilmente una coincidenza e solleva la necessità di conduzione di una ricerca sistematica per la CCSVI in pazienti affetti da SLA.
